چکیده
Introduction: Tuberculosis (TB) remains one of the world's deadliest infectious diseases, and smear-positive pulmonary tuberculosis (SPPTB) is the most contagious form. The Iranshahr province, Iran, bordering high TB-burden countries such as Pakistan and Afghanistan, has long been considered an endemic area. The aim of this study was to determine the epidemiology and incidence of SPPTB in this endemic region. Materials and Methods: A retrospective cross-sectional study was conducted using registry-based data of all suspected and confirmed TB patients in Iranshahr city from 2021 to 2024. Information was collected using a standard checklist including demographic and clinical information features and World Health Organization (WHO) TB patients’ classification. Descriptive analyses for the study were performedusing SPSS software. Results: Among 3,078 persons evaluated for TB, 298 (9.7%) were smear-positive, 287 (96.3%) were confirmed as smear positive pulmonary TB (SPPTB) by GeneXpert testing, and 283 (95.0%) patients were detected as SPPTB by culture. The mean ± SD age of SPPTB patients was 49.20 ± 19.43 years (range 14-106 years) with 154 males (53.7%) compared to 133 females (46.3%). The highest proportion of SPPTB cases occurred in the 31–60-year age group, followed by the ≥61-year age group. Most patients were Iranian (279, 97.2%), resided in urban areas (195 patients, 67.9%), and were married (249 patients, 86.8%). Conclusion: SPPTB continues to be a significant public health concern in southeastern Iran, predominantly affecting middle-aged adults, men, and urban residents. Our results emphasize the impact of demographic factors and cross-border migration on TB transmission.
Epidemiology of smear-positive pulmonary tuberculosis: A retrospective cross‐sectional registry-based study

سل (TB) همچنان یکی از کشندهترین بیماریهای عفونی جهان است و سل ریوی اسمیر مثبت (SPPTB) مسریترین نوع آن است. استان ایرانشهر، ایران، که با کشورهای با بار بالای سل مانند پاکستان و افغانستان هممرز است به عنوان یک منطقه بومی در نظر گرفته میشود. هدف از این مطالعه تعیین اپیدمیولوژی و میزان بروز SPPTB در این منطقه بود. یک مطالعه مقطعی گذشتهنگر با استفاده از دادههای مبتنی بر ثبت تمام بیماران مشکوک و تایید شده سل در شهرستان ایرانشهر از سال 2021 تا 2024 انجام شد. اطلاعات با استفاده از یک چک لیست استاندارد جمعآوری شد. از بین 3078 نفری که برای سل ارزیابی شدند، 298 نفر (9.7٪) اسمیر مثبت بودند، 287 نفر (96.3٪) با آزمایش GeneXpert به عنوان سل ریوی اسمیر مثبت (SPPTB) تأیید شدند و 283 بیمار (95.0٪) با کشت به عنوان SPPTB تشخیص داده شدند. میانگین ± انحراف معیار سنی بیماران SPPTB 49.20 ± 19.43 سال (محدوده 14 تا 106 سال) بود که 154 نفر مرد (53.7٪) در مقایسه با 133 نفر زن (46.3٪) بودند. بیشترین نسبت موارد SPPTB در گروه سنی 31 تا 60 سال و پس از آن گروه سنی ≥61 سال رخ داد. اکثر بیماران ایرانی (279، 97.2٪)، ساکن مناطق شهری (195 بیمار، 67.9٪) و متاهل (249 بیمار، 86.8٪) بودند. سل ریوی (SPPTB) همچنان یک نگرانی مهم بهداشت عمومی در جنوب شرقی ایران است که عمدتاً بزرگسالان میانسال، مردان و ساکنان شهری را تحت تأثیر قرار میدهد. نتایج ما بر تأثیر عوامل جمعیتی و مهاجرت فرامرزی بر انتقال سل تأکید دارد.
