چکیده
Introduction: Colorectal cancer (CRC) is the third most commonly diagnosed malignancy and the second leading cause of cancer-related mortality worldwide. According to GLOBOCAN projections, the global burden of CRC is expected to affect approximately 3.2 million individuals by 2040. This study aims to evaluate the temporal trends in the burden of CRC in Iran by analyzing disability-adjusted life years (DALYs) from 1995 to 2019.
Materials & Methods: Data were retrospectively obtained from the global burden of disease (GBD) reports, published by the Institute for Health Metrics and Evaluation (IHME). The burden of CRC was assessed using DALYs, incidence, and prevalence rates, stratified by sex and age group over the study period. Statistical significance was determined using the Jonckheere-Terpstra test and t-tests at a 5% significance level.
Results: Between 1995 and 2019, CRC accounted for approximately 2.4 million DALYs in Iran, with males contributing 1,330,961 DALYs and females 1,095,938 DALYs. The incidence rate increased from 4.63 per 100,000 population during 1995–1999 to 10.85 per 100,000 during 2015–2019, while the prevalence rate rose from 19.32 to 49.45 per 100,000 population. Over the same period, a total of 94,188 CRC-related deaths were recorded, with a higher mortality rate observed among males compared to females.
Conclusion: The findings demonstrate significant variations in CRC burden across different age and sex groups. CRC remains a major public health concern, with increasing incidence and prevalence. The majority of DALYs were attributable to years of life lost (YLLs) due to premature mortality, highlighting the critical need for early detection and preventive strategies.

بار سرطان کولورکتال در ایران طی سال های 1995 تا 2019: نتایجی از مطالعه بار بیماری ها

سرطان کولورکتال به عنوان سومین سرطان شایع و دومین علت مرگ ناشی از سرطان شناخته شده است. بر اساس گزارشات GLOBOCAN، تعداد مبتلایان به سرطان کولورکتال در سال 2040 به 2/3 میلیون نفر خواهد رسید. هدف از این مطالعه، بررسی بار بیماری سرطان کولورکتال در ایران بر اساس نتایج مطالعه بار بیماری‌ها در بازه زمانی 1995 تا 2019 است. داده‌ها به صورت گذشته‌نگر از مطالعه بار بیماری‌ها جمع‌آوری شده‌اند. برای بررسی بار بیماری سرطان کولورکتال، از شاخص‌های سال‌های از دست رفته عمر، میزان بروز و میزان شیوع استفاده شده است. بنابراین، این شاخص‌ها در بازه زمانی 1995 تا 2019 بر اساس گروه‌های سنی و جنسی برآورد شده‌اند. برای بررسی تفاوت بار بیماری سرطان کولورکتال بر اساس گروه‌های سنی و جنسی، از آزمون تی تست و برای بررسی روند بار بیماری سرطان کولورکتال، از آزمون Cochran-Armitage استفاده شده است. از سال 1995 تا 2019، تقریبا 4/2 میلیون سال از دست رفته عمر به دلیل سرطان کولورکتال در ایران وجود داشته است. میزان بروز سرطان کولورکتال از 3/4 نفر در سال 1995 به 7/11 نفر به ازای هر 100000 نفر در سال 2019 افزایش یافته است. میزان شیوع نیز از 18 به 9/54 افزایش یافته است. نتایج مطالعه نشان می‌دهد که بار بیماری سرطان کولورکتال بر اساس گروه‌های سنی و جنسی تفاوت معنی‌داری در سطح 5 درصد دارد. با افزایش شیوع سرطان کولورکتال، این بیماری به یک مشکل مهم جهانی تبدیل شده است. با توجه به اینکه قسمت عمده سال‌های از دست رفته عمر در سرطان کولورکتال به دلیل مرگ‌های زودرس است، بنابراین اجرای برنامه‌های غربالگری و تشخیص زودرس بیماری اهمیت زیادی دارد.
